21 And the Lord shall be known to Egypt, and the Egyptians shall know the Lord in that day, and shall do sacrifice and oblation; yea, they shall vow a vow unto the Lord, and perform it.

22 And the Lord shall smite Egypt: he shall smite and heal it: and they shall return even to the Lord, and he shall be intreated of them, and shall heal them.

23 In that day shall there be a highway out of Egypt to Assyria, and the Assyrian shall come into Egypt, and the Egyptian into Assyria, and the Egyptians shall serve with the Assyrians.

24 In that day shall Israel be the third with Egypt and with Assyria, even a blessing in the midst of the land:

25 Whom the Lord of hosts shall bless, saying, Blessed be Egypt my people, and Assyria the work of my hands, and Israel mine inheritance.
Isaiah 19:19-25

My question is when or did this already happen? Or is this a reference to the millenium when Christ rules on the earth?

This part and shall do sacrifice and oblation; shows that it already happened since we no longer do sacrifices.

I believe this is future. One day Egypt and Syria will walk with Israel and the Elect in following Jesus, the King.

Maybe in the millenial because it talks of the total destruction of Damascus which is in Syria. By this we know that they will be aligned against Israel in the Tribulation or perhaps as a forerunner to it

I cannot imagine how it would be possible to argue that the above has already happened.

And we offer the Holy Oblation in Peace...

You could take that approach, but I see the verses as referring to Egypt overall, or Egypt officially, not to a tiny minority of its people. In that case, it has not happened yet.

For example "the Egyptians shall know the Lord in that day,..." This does not mean that a few Egyptians in the midst of a hostile majority Egyptian populace shall know....

If you take it to mean a literal animal sacrifice that is true, but "sacrifice" can be interpreted other ways also
