If they catch it at the beginning, yes, they can understand it. My mother-in-law had dementia and was embarrassed by it.

In the early stages, probably. In the later stages, almost certainly not.

The trouble with early stages is telling the difference between normal aging and something that might need attention.

In the beginning, yes. As it progresses, they do not. Have taken care of many dementia patients over the years. They may have a lucid moment once in a while, but those tend to not happen very often.
