A health care directive

A health care directive is a written document that informs other of your wishes about your health care. It allows you to name a person ("agent") to decide for you if you are unable to decide. It also allows you to name an agent if you want someone else to decide for you.

Yes we do. Ours name a person as a power of attorney for health care. My family understands my wishes though the directive it gives the POA the ability to make a decision based on all information give by the providing physicians as I feel I can't say if "X" happens then do "Z", when in reality "Y" might be the best way.

Basically, ours gives the POA the right to make decisions after consulting with the siblings and the treating physicians.

No, I don't. When I was a hospital chaplain we use to help people do these all the time. We called them medical power of attorneys if the person is unable to make decisions themselves.
