It depends how you define "worth it".

My wife has a basic one. It tracks her steps, pulse, activities etc, and the software also lets her monitor everything from her water intake to her monthly cycle. Obviously she decides how much information to give it.

I have a more advanced model because I do more running and want something that will track more fitness metrics. I think mine would monitor a monthly cycle but since I don't have one it doesn't show up in the software. It measures things like my breathing rate, sleep quality etc. When I'm exercising it also measures things like my performance condition (whether I'm training above, at or below my normal standard) and can do things like plan a pace for a predefined course based on hills and whether I want to start fast and ease off or start slow and speed u p.

For the most part I think it's worth it, although some of the metrics it provides don't feel very accurate. I have days when I toss and turn all night but my smartwatch says I slept quite well, and there are times I'm watching TV in the evening when it thinks I've gone to sleep. In fairness all it can do is measure movement and my pulse/breathing, so if I'm motionless and my pulse slows it may be hard to tell the difference between that and being asleep.

For me they are worth it. I am able to get pulse readings, an ECG reading and my oxygen saturation which are all important for me to keep track of being a cardiac patient. It also shows me what I have done as far as working out.
