No, I don't wet my hands first because then the soap would glide off into the sink. I put it on dry and lather up (probably not the full 20 seconds I'm supposed to) and then rinse off thoroughly.

I mostly use foaming soap so wet my hands, add a pump of soap, then wash. How long I wash them depends on how dirty they are - if it's a simple routine matter of washing hands before handling food it's usually fairly brief but if my hands and forearms are blackened with dirt from working on the house then I'll take as long as I need to get them clean. It's pretty grim when the water coming off my hands is dark gray.
