Personally I'd say when it's either legally required or someone's life is in danger.
My standard line is that if people want to tell me something in confidence then it stays in confidence but they should know I'm a mandated reporter so if it creates a suspicion of child abuse I'm legally required to report, and if they or someone else is in immediate danger I'll call emergency services.
